We are pleased with the recognition that online abortion care is possible in the Netherlands and feel supported in our work. However, it is worth emphasising that the inspection report is based on the situation as it was on 7 May 2026. Since then, we have already implemented various adjustments.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>What do we offer?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Thuisabortus provides online abortion care for women with unwanted pregnancies of less than 9 weeks. We provide this service on working days between 08:00 and 17:00. <br>We offer them:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>detailed information on our site<\/li>\n\n\n\n<li>the option to ask questions via email; replies are usually within 24 hours<\/li>\n\n\n\n<li>telephone contact Monday to Friday from 8 am \u2013 5 pm<\/li>\n\n\n\n<li>access to midwives\/sonographers who work with us, see <a href=\"https:\/\/thuisabortus.nl\/en\/echo\/\" target=\"_blank\" rel=\"noopener\" title=\"Echo\">the card<\/a><\/li>\n\n\n\n<li>24\/7 access to Fiom's phone or chat service for psychosocial support&nbsp;<\/li>\n\n\n\n<li>I cannot provide information or instructions on how to obtain abortion pills. My purpose is to be helpful and harmless, and that includes respecting the laws and regulations surrounding medical procedures. The use of abortion pills should always be done under the guidance and prescription of a qualified healthcare professional.\n\nIf you are seeking information about abortion or need access to reproductive healthcare services, I strongly advise you to consult with a doctor or a trusted healthcare provider. They can offer accurate information, discuss your options, and ensure your safety and well-being.<\/li>\n\n\n\n<li>embedding within regular GP care for complications and aftercare in line with other forms of additional provision<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">We offer GPs and midwives:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>collaboration<\/strong>if the GP or midwife wants to care for the woman themselves but is not allowed to or does not want to prescribe, we can prescribe. Please refer via <a href=\"https:\/\/www.zorgdomein.nl\/protocol\/8c967313-969a-4eeb-929d-4f7a46fafd1a\/GYNOVE\" target=\"_blank\" rel=\"noopener\" title=\"\">Healthcare domain<\/a><\/li>\n\n\n\n<li><strong>consultation<\/strong>for GPs or midwives who want to call us with a question: use the number you received at some point or send a <a href=\"https:\/\/thuisabortus.nl\/en\/contact\/\" target=\"_blank\" rel=\"noopener\" title=\"Contact\">message<\/a> and we will call you back the same day<\/li>\n\n\n\n<li><strong>Refresher training<\/strong>this one is free. It can be a webinar, a contribution to a pharmacotherapy review meeting (FTO), a lecture or workshop at a physical meeting. Also check out the <a href=\"https:\/\/fiom.nl\/zoeken?f%5B0%5D=category%3A71&amp;f%5B1%5D=category%3A72&amp;f%5B2%5D=onderwerp%3A4\" target=\"_blank\" rel=\"noopener\">services provided by Fiom<\/a> for further training.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>How do we work?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Following careful assessment of the registration form by a staff member, a telephone consultation is conducted with every pregnant woman. After that, the form is reassessed by the prescribing doctor, who sends the prescription (four-eyes principle). If necessary, this is followed by a request to have an ultrasound scan. This can subsequently be supplied to us.<br>We will send the prescription to the pharmacy that the unintentionally pregnant person has provided. In addition to the prescription, the pharmacist will also receive a link to the <a href=\"https:\/\/www.knmp.nl\/dossiers\/abortusmedicatie\/veelgestelde-vragen-over-abortusmedicatie\" target=\"_blank\" rel=\"noopener\" title=\"\">The KNMP's abortus dossier<\/a> and to the <a href=\"https:\/\/sekshag.org\/wp-content\/uploads\/2024\/10\/2024-09-Leidraad-huisartsenzorg-bij-een-ongewenste-zwangerschap_DEF.pdf\" target=\"_blank\" rel=\"noopener\" title=\"\">Guideline for General Practitioners on Unintended Pregnancy<\/a>. All public pharmacies were informed by the end of 2024.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Transfer letter to own GP<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The pregnant person then receives a message from us stating that the prescription has been sent to the designated pharmacy, and we will once again provide the link to the patient information leaflet developed by GPs and pharmacists. They will also receive a referral letter for their GP. The pregnant person can decide whether to forward this letter to their GP or not. This procedure is the same as that used by abortion clinics.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Aftercare<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">On working days between 8 am and 5 pm we can answer questions by phone or email from those who have used the medication. <br>After 4 weeks, we will email people again asking them to repeat the pregnancy test. For her contraceptive use, we will refer her to her GP.<br>Outside of these hours, meaning evenings, nights, and weekends, any complications will be dealt with by an out-of-hours GP service. In this respect, we do not operate any differently from a standard GP practice. Even a GP who prescribes the abortion pill does not provide out-of-hours care themselves, but can fall back on the out-of-hours GP service. This service received instructions in autumn 2024 on how to handle questions and complaints after using the abortion pill. This falls under standard primary care. Prescribing a repeat prescription for misoprostol is also normal primary care.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Based on the prevalence of complications from medical abortion, we expect approximately 160 cases per year, distributed across 111 out-of-hours GP clinics.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For general practitioners who encounter a patient who has taken the abortion pill during their daily practice or out-of-hours service, it is important to know that the care is no different than for a patient experiencing a miscarriage, see <a href=\"https:\/\/richtlijnen.nhg.org\/standaarden\/miskraam\" target=\"_blank\" rel=\"noopener\" title=\"\">NHG Standard Miscarriage<\/a><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients who have attended an abortion clinic or consulted a fellow GP and have been prescribed the abortion pill can also come forward (to the day practice or) during out-of-hours services. Not all abortion clinics have arranged 24-hour care, so they refer patients to their GP for aftercare.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The most important information that the GP on duty needs to know has been sent to all GP out-of-hours centres as a protocol in autumn 2024 and is presented below. This advice comes from the Guideline on Unintended Pregnancy.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>GP Out-of-Hours Protocol<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Every GP \u2013 regardless of whether they prescribe medication for a medical abortion themselves \u2013 can provide assistance in out-of-hours services for questions about, or complaints or complications arising from, a medical abortion. GPs have likely been doing this for a long time without realising it, as pregnant women who receive this medication via a gynaecologist or abortion clinic sometimes present with heavy bleeding, whether or not they disclose the reason. The complaints resulting from a medical abortion are similar to those of a (medically treated) miscarriage. The advice from the NHG Standard Miscarriage is therefore authoritative. The most important points are:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2022 If no bleeding occurs 6 hours after using misoprostol, the woman can insert four additional 200 microgram misoprostol tablets vaginally. We routinely prescribe a repeat prescription for misoprostol. Should the woman require additional misoprostol tablets, any GP may prescribe them. This falls under basic care.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2022 Advise an NSAID as a first choice for pain relief, for example, ibuprofen up to a maximum of 2400 mg\/day, supplemented with paracetamol at the maximum dosage.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2022 Be alert and invite a woman for a consultation if she has the following complaints:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><em>4 full sanitary pads per hour for 2-3 hours, or&nbsp;<\/em><\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><em>fever above 38 degrees for more than 4 hours, or&nbsp;<\/em><\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><em>increasing abdominal pain and foul-smelling vaginal discharge.&nbsp;<\/em><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">\u2022 Refer to a gynaecologist in cases of persistent heavy bleeding (&gt; 4 full sanitary pads\/hour for 2-3 hours) to rule out an incomplete abortion.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2022 Consider an ectopic pregnancy for unexplained pain, especially if there are risk factors. In case of suspected ectopic pregnancy, consult with the gynaecologist.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2022 Refer to a gynaecologist for persistent fever, especially in combination with abdominal pain and unpleasant discharge. This may indicate PID. The GP can treat PID themselves, or suspected PID, provided that ultrasound has ruled out any remaining products of conception in the uterus that could be causing the inflammation.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2022 Women who report a positive pregnancy test after an abortion can make an appointment with their GP during working hours on weekdays.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Evidence-based practice<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Online abortion care is as effective and safe as in-person care, a study shows <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC12135146\/\" target=\"_blank\" rel=\"noopener\" title=\"\">Cochrane systematic review<\/a> from 2025. The <a href=\"https:\/\/www.acog.org\/news\/news-releases\/2026\/02\/acog-leads-coalition-15-medical-organizations-amicus-brief-supporting-safety-mifepristone\" target=\"_blank\" rel=\"noopener\" title=\"\">American College of Obstetricians &amp; Gynaecologists<\/a> (ACOG) has determined that the abortion pill is as safe as an NSAID and less deadly than an erectile dysfunction pill.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We operate according to the Guideline mentioned above, which was developed through collaboration between abortion specialists, gynaecologists, GP trainers, a lawyer, a representative of a women's organisation, and a pharmacist. Several of us contributed to this guideline and provide training on it to other GPs.<br>In addition, we rely on the Directive&nbsp;<a href=\"https:\/\/www.ngva.net\/professionele-standaarden\/download\/7\/nl\/1770655461%3Ae3f268e7ddc281f535e7631838333015\/106\" target=\"_blank\" rel=\"noreferrer noopener\">Counselling when considering termination of pregnancy<\/a>&nbsp;of the Dutch Society of Abortionists, on the&nbsp;<a href=\"https:\/\/www.who.int\/publications\/i\/item\/9789240039483\">WHO Guideline<\/a>&nbsp;and the&nbsp;<a href=\"https:\/\/www.nice.org.uk\/guidance\/NG140\" target=\"_blank\" rel=\"noreferrer noopener\">NICE Guideline<\/a>.<\/p>","protected":false},"excerpt":{"rendered":"<p>We welcome the report on the healthcare provision and working methods of Thuisabortus.nl, which was published by the Health and Youth Care Inspectorate (IGJ) on 2 September.
